About the role

Key responsibilities & impact
  • Examine and treat patients under the direction of the physician
  • Review patient medical records
  • Interview patients and record vital signs
  • Provide nursing interventions and coaching in accordance with scope of practice and standing orders
  • Facilitate patient transfers to an alternate level of care as needed
  • Document using standard templates
  • Engage with patients by phone for up to 80% of the shift
  • Work assigned weekends according to the standard team rotation
  • Complete required in-person training sessions
  • Work the initial two-week Monday-Friday training schedule, then the regular four-10-hour shift schedule

Requirements

What you’ll need
  • LPN or LVN qualification
  • Minimum of 2 years of experience as an LPN/LVN
  • Current LPN/LVN license in the applicable state required
  • Satisfactory completion of a Tuberculosis test required for the Carelon Health business unit
  • Reliable internet
  • High level of customer service
  • Ability to engage with patients by phone for up to 80% of the shift
  • Ability to work assigned weekends according to standard team rotation
  • Ability to work holidays and weekend rotation
  • Bi-lingual (English/Spanish) preferred
  • Experience with Telephonic Triage preferred
  • Acute care experience, such as urgent care or ER, preferred
  • Prior Med surg or clinic experience preferred
  • Experience treating and educating individuals with diabetes preferred
