About the role

Key responsibilities & impact

Examine and treat patients under the direction of the physician
Review patient medical records
Interview patients and record vital signs
Provide nursing interventions and coaching in accordance with scope of practice and standing orders
Facilitate patient transfers to an alternate level of care as needed
Document using standard templates
Engage with patients by phone for up to 80% of the shift
Work assigned weekends according to the standard team rotation
Complete required in-person training sessions
Work the initial two-week Monday-Friday training schedule, then the regular four-10-hour shift schedule

Requirements

What you’ll need

LPN or LVN qualification
Minimum of 2 years of experience as an LPN/LVN
Current LPN/LVN license in the applicable state required
Satisfactory completion of a Tuberculosis test required for the Carelon Health business unit
Reliable internet
High level of customer service
Ability to engage with patients by phone for up to 80% of the shift
Ability to work assigned weekends according to standard team rotation
Ability to work holidays and weekend rotation
Bi-lingual (English/Spanish) preferred
Experience with Telephonic Triage preferred
Acute care experience, such as urgent care or ER, preferred
Prior Med surg or clinic experience preferred
Experience treating and educating individuals with diabetes preferred
