About the role

Key responsibilities & impact

Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
Provide clinical and non-clinical support to patients in palliative care within a designated territory
Provide services in home settings to care for patients
Develop and implement clinical plans of care for adult patients with chronic and complex conditions
Gather histories, perform physical exams, and obtain diagnostics as needed
Develop and implement treatment plans based on patients’ goals of care and current conditions
Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis prevention
Coordinate care and resources with clinical team members, physicians, and medical providers
Maintain patient medical records and medical documentation in accordance with regulations and company standards
Participate in continuing education
Prescribe medication as permitted by state prescribing authority

Requirements

What you’ll need

Requires an MS in Nursing
Requires current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
Possession of DEA registration or eligibility required
CPR certification required
2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred
Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience highly preferred
Experience working with Electronic Medical Records (EMR) preferred
Bilingual in English/Spanish preferred
Satisfactory completion of a Tuberculosis test required for the Carelon Health business unit
Travel may be required
