About the role

Key responsibilities & impact
  • Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
  • Provide clinical and non-clinical support to patients in palliative care within a designated territory
  • Provide services in home settings to care for patients
  • Develop and implement clinical plans of care for adult patients with chronic and complex conditions
  • Gather histories, perform physical exams, and obtain diagnostics as needed
  • Develop and implement treatment plans based on patients’ goals of care and current conditions
  • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis prevention
  • Coordinate care and resources with clinical team members, physicians, and medical providers
  • Maintain patient medical records and medical documentation in accordance with regulations and company standards
  • Participate in continuing education
  • Prescribe medication as permitted by state prescribing authority

Requirements

What you’ll need
  • Requires an MS in Nursing
  • Requires current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
  • Possession of DEA registration or eligibility required
  • CPR certification required
  • 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred
  • Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience highly preferred
  • Experience working with Electronic Medical Records (EMR) preferred
  • Bilingual in English/Spanish preferred
  • Satisfactory completion of a Tuberculosis test required for the Carelon Health business unit
  • Travel may be required
  • Candidates must be within a reasonable commuting distance from the posting location(s), unless accommodation is granted as required by law
  • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
