About the role
Key responsibilities & impact- Work under the direction and supervision of an RN with overall responsibility for the member's case as required by applicable state law and contract
- Contribute to the LTSS care coordination process within the scope of licensure
- Assist the responsible RN with telephonic or face-to-face assessments to identify, evaluate, coordinate, and manage members' physical health, behavioral health, social services, and long-term services and supports needs
- Assist the responsible RN in identifying members at high risk for complications
- Obtain clinical data as directed by the responsible RN
- Assist in identifying members who may benefit from an alternative level of care or waiver programs
- Provide collected information to the responsible RN for verification, interpretation, additional assessments, and care-plan development
- Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities
- Collaborate with the RN, member, and health care team to support cost-effective and efficient utilization of health benefits
- Apply clinical guidelines and understand disease processes and terminology without exercising nursing judgment
- Travel to client sites, worksites, and other locations as necessary
Requirements
What you’ll need- LPN/LVN, LSW, LCSW, LMSW, or another non-RN license in accordance with applicable state law
- Nursing Diploma or Associate of Science in Nursing or a related field
- Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role, or an equivalent combination of education and experience
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or another non-RN license as allowed by state law in applicable state(s)
- May require state-specified certification based on state law and/or contract
- Candidate should reside in Johnson County, Indiana and Shelby County, Indiana
- Ability to work in a field-based role and travel to client sites or designated locations
- Preferred: MA/MS in social work
- Travel to the worksite and other locations as necessary preferred
