About the role

Key responsibilities & impact

Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
Provide clinical and non-clinical support to patients
Provide urgent healthcare by telephone and tele-video to patients receiving home and community-based services
Develop and implement clinical care plans for adults with chronic and complex conditions
Gather medical histories, conduct physical examinations, and obtain diagnostics as needed
Develop and implement treatment plans based on patients’ goals and conditions
Identify and close gaps in care
Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
Educate patients and families about medications, side effects, illness progression, diet and nutrition, adherence, and crisis prevention
Coordinate care and resources with clinical team members and medical providers
Maintain patient medical records and documentation in accordance with regulations and company standards
Participate in required continuing education
Prescribe medication as permitted by state authority

Requirements

What you’ll need

Master of Science in Nursing (MSN)
Active national Nurse Practitioner certification
Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Texas, Iowa, or Kansas
Valid, current, active RN Compact license
2+ years of experience managing complex care cases
Experience working with Electronic Medical Records (EMR)
Monday-Friday availability, 8:00am-5:00pm CST, with rotating on-call
Possession of DEA registration or eligibility preferred
Active Medicaid number preferred
Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
