About the role

Key responsibilities & impact

Responsible for overall management of member's individual service plan within the scope of position
Develops, monitors, and revises the member's service plan to meet the member's needs
Responsible for performing telephonic or face-to-face history and program needs assessments
Identifies members with potential clinical health care needs and coordinates those member's cases
Manages non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
Establishes short and long-term service and support goals in collaboration with the member, caregivers, family, natural supports, physicians
May also serve as a mentor or preceptor for new staff

Requirements

What you’ll need

Requires a BA/BS degree
Minimum of 2 years of experience working with a social work agency
Minimum of 5 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role
Bilingual in Spanish/English preferred
Specific education and years and type of experience may be required based upon state law and contract requirements
