Health Coach
Elevance HealthHealth Coach providing virtual health coaching to members with chronic diseases. Responsible for conducting assessments, coordinating coaching, and implementing behavior change plans.
About the role
Key responsibilities & impact- Provides health coaching to members with chronic diseases
- Conducts behavioral assessments to identify individual member knowledge, skills, and behavioral needs
- Coordinates health coaching as directed by nurse case manager to address objectives and goals
- Implements coaching plans using behavior change principles
- Monitors and evaluates interventions and modifies as necessary.
Requirements
What you’ll need- Requires BA/BS in appropriate field of specialization (e.g., Health Education, Exercise Physiology, Respiratory Therapy or Dietician)
- Minimum of 3 years of related experience in health education, exercise instruction, or patient education
- Certification, advanced certification, and/or license appropriate to field of specialty as required
- Preferred experience in health coaching, disease management, knowledge of behavioral and/or clinical assessment techniques, health and/or patient education and behavior change techniques
- Understanding of disease management principles.
