Key responsibilities & impact

Perform care management activities for members with complex and chronic care needs
Ensure high quality, cost-effective care and appropriateness of treatment
Partner with physicians and/or pharmacists to interpret appropriateness of care
Collaborate with providers to assess consumer needs and plan for adverse events
Conduct clinical assessments to develop individual care plans and goals
Monitor and evaluate effectiveness of care management plans

Requirements

What you’ll need

Requires an ASN or ADN and a minimum of 3 years of clinical experience
Current, valid, active unrestricted RN license in applicable state(s) required
Multi-state licensure is required for services in multiple states
Preferred: BSN, Specialty Pharmacy experience, Bilingual, Drug Manufacturer experience, Telephonic Nursing experience
