RN Case Manager – Telephonic, Field
Elevance HealthRN Case Manager responsible for care management for members with complex needs. Conducts assessments and coordinates care plans for Kansas Medicaid members in South Central and South West Kansas.
About the role
Key responsibilities & impact- Responsible for performing care management within the scope of licensure for members with complex and chronic care needs
- Assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum
- Conduct assessments and coordination of care telephonically and occasionally on-site
- Ensure member access to services appropriate to their health needs
- Conduct assessments to identify individual needs and develop care management plans
- Implement care plans by facilitating authorizations/referrals as appropriate
- Coordinate internal and external resources to meet identified needs
- Monitor and evaluate effectiveness of care management plans and modify as necessary
- Interface with Medical Directors and Physician Advisors on care management treatment plans
Requirements
What you’ll need- Requires BA/BS in a health-related field and minimum of 3 years of clinical experience
- Current, unrestricted RN license in the state of Kansas is required
- Computer proficiency strongly preferred
