Transitions of Care RN – Bilingual Preferred
Elevance HealthTransitions of Care RN delivering patient education and health coaching services remotely. Responsible for clinical assessments, care plan coordination, and behavioral health referrals.
About the role
Key responsibilities & impact- Participating in delivery of patient education and disease management interventions
- Performing health coaching for members
- Conducting behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs
- Identifying and/or coordinating specific health coaching plan needs
- Interfaces with provider and other health professionals for health coaching plan coordination
- Implementing and/or coordinating coaching and/or care plans
- Using motivational interviewing to facilitate health behavior change
- Monitoring and evaluating effectiveness of interventions
Requirements
What you’ll need- Requires AS in nursing
- minimum of 2 years of condition specific clinical or home health/discharge planning experience
- Current unrestricted RN license in applicable state(s) required
- RN compact license is highly preferred
- BS in nursing preferred
- Prior case management experience preferred
- Bilingual in Spanish is highly preferred
- Experience working with members that have chronic diseases is highly preferred
- Experience working in home health is preferred
