Key responsibilities & impact

Perform case management telephonically and/or by home visits
Use screening criteria knowledge and clinical judgment to assess member needs
Conduct assessments to identify individual needs and develop care plan
Monitor and evaluate effectiveness of care plan and modify as needed
Support member access to appropriate quality and cost effective care

Requirements

What you’ll need

MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing
Minimum of 3 years clinical experience in social work counseling with complex psychiatric and substance abuse treatment
Current active unrestricted license (RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist) as required by applicable state laws
