Manage and oversee assigned care managers and ensuring fidelity to the CFSP Care Management model which includes physical health, behavioral health, and social services.
Manage resource utilization to ensure appropriate delivery of care to members, adequate coverage for all tasks and job responsibilities.
Review all Care Plans and ISPs for quality control and provide guidance to care managers on how to address Members’ complex health and social needs.
Ensure care managers provide Trauma-Informed Care and recognize the impact of ACEs on the CFSP population.
Coordinate service delivery to include member assessment of physical and psychological factors.
Participate in cross-functional workgroups created to maintain and develop program.
Evaluate current processes of Special Program's support functions; recommend changes for increased efficiencies and improved outcomes.
Develop and conduct training programs for staff involved in the program.
Extract and manipulate analytical data to present findings to relevant markets and stakeholders.
Hire, train, coach, counsel, and evaluate performance of direct reports.
Travel within Haywood/Jackson/Swain counties; work virtually from home when not in the field.
Requirements
NC RESIDENCY IS REQUIRED
Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina.
Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
Requires three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.
At least 2 years of management/supervisor experience (with direct reports) is needed for this position.
