About the role
- Manage and oversee assigned care managers and ensuring fidelity to the CFSP Care Management model which includes physical health, behavioral health, and social services.
- Manage resource utilization to ensure appropriate delivery of care to members, adequate coverage for all tasks and job responsibilities.
- Review all Care Plans and ISPs for quality control and provide guidance to care managers on how to address Members’ complex health and social needs.
- Ensure care managers provide Trauma-Informed Care and recognize the impact of ACEs on the CFSP population.
- Coordinate service delivery to include member assessment of physical and psychological factors.
- Participate in cross-functional workgroups created to maintain and develop program.
- Evaluate current processes of Special Program's support functions; recommend changes for increased efficiencies and improved outcomes.
- Develop and conduct training programs for staff involved in the program.
- Extract and manipulate analytical data to present findings to relevant markets and stakeholders.
- Hire, train, coach, counsel, and evaluate performance of direct reports.
- Travel within Haywood/Jackson/Swain counties; work virtually from home when not in the field.
Requirements
- NC RESIDENCY IS REQUIRED
- Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina.
- Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
- Requires three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.
- At least 2 years of management/supervisor experience (with direct reports) is needed for this position.
- We are unable to accommodate LCSW-A, LCMHC-A or any other associate level licenses.
