Key responsibilities & impact

Serves as a key connector between underserved communities and health/social service systems
Focuses on promoting health equity and improving care coordination
Responsible for outreach, community member or patient advocacy and care coordination
Builds trusting relationships to mediate between individuals and health/social service systems
Identifies and engages individuals and families through outreach activities and community events
Evaluates the specific needs and challenges faced by community members
Identifies barriers such as housing and food insecurity and connects individuals to appropriate resources
Applies specialized knowledge to provide targeted education and resource navigation
May conduct outreach activities, including home visits
Maintains accurate and up-to-date records to support advocacy efforts

Requirements

What you’ll need

High School Diploma or equivalent (required)
Bachelor's degree in Social Work, Public Health, Psychology, or related field (preferred)
Community Health Worker (CHW) Certification in the state of practice (required)
Valid driver's license with good driving record (required, South Carolina only)
1 year experience as a Community Health Worker (required)
3 years of recent experience in a community outreach or healthcare setting (preferred)
Proficient with computers and accuracy with data entry and Microsoft Office
Comprehensive knowledge of the field's concepts and principles
Sensitivity and experience in working within different cultures
Cultural competency and ability to work with diverse populations
