About the role

Key responsibilities & impact

provide clinical nursing, administrative and organizational skills in managing continuity of care
conduct comprehensive assessments of Active-Duty Service Members' health needs
facilitate communication and coordination between members of the healthcare team
empower patients and serve as an advocate for both patients and the healthcare team
participate in all phases of the Case Management Program (CMP) and ensure standards of care are met
accurately collect and document patient care data in electronic healthcare record
develop and implement tools to support case management

Requirements

What you’ll need

Baccalaureate Degree of Nursing
one year of nursing experience
current, full, active, and unrestricted license to practice as a Registered Nurse
certifications including CCM, CDMS, CRRN, COHN, COHN-S, ACCC, CRC, RN-NCM, CMC
