About the role

Key responsibilities & impact
  • Conduct initial and follow-up nutrition assessments
  • Develop and revise individualized nutrition care plans with patients
  • Provide medical nutrition therapy and nutrition interventions for chronic disease risk
  • Educate patients on healthy nutrition, diabetes, prediabetes, and blood glucose management
  • Document patient encounters and care plans in the electronic medical record
  • Select and produce culturally sensitive nutrition education materials
  • Develop and implement nutrition education materials, curricula, groups, policies, procedures, protocols, and templates
  • Assist with the Diabetes Education Accreditation Program process
  • Collaborate with and refer patients to external services, programs, agencies, and providers
  • Coordinate with internal departments to connect patients with additional services
  • Monitor appointments and reschedule missed appointments
  • Maintain community resources and the nutrition referral network
  • Serve as a nutrition resource for practitioners and interdisciplinary care teams
  • Manage caseload and service utilization using reporting tools
  • Participate in interdisciplinary team meetings and discuss complex patient cases
  • Support patients with health improvement and self-management
  • Provide care within applicable scope of practice and established standards
  • Attend meetings, trainings, huddles, and program subcommittees
  • Travel to DAP Health locations throughout San Diego and Riverside

Requirements

What you’ll need
  • Bachelor’s degree in nutrition, dietetics, public health, or related field
  • Registered by the Commission on Dietetic Registration, or eligible to obtain registration within 6 months
  • Experience delivering nutrition counseling via telehealth platforms
  • Current BLS certification from the American Heart Association, American Red Cross, or American Safety and Health Institute
  • Knowledge of public health, prediabetes, and diabetes self-management concepts preferred
  • Ability to develop normal and therapeutic diets aligned with cultural and religious needs and preferences
  • Ability to communicate effectively with diverse cultural and ethnic groups
  • Experience with motivational interviewing
  • Excellent oral and written communication skills
  • Effective interpersonal skills
  • Ability to work in team-based and independent environments
  • Basic computer literacy, including electronic medical record documentation
  • Ability to prioritize and manage multiple tasks in a fast-paced environment
  • Bilingual English/Spanish preferred
  • Knowledge of nutrition considerations for individuals living with HIV
  • Current and valid driver’s license and personal auto insurance
  • Ability to lift/move up to 40 pounds, stand for long periods, and travel between DAP Health locations
  • Experience working at an FQHC and with underserved, uninsured, and minority populations preferred
  • Experience with the Diabetes Prevention Program and CDCES credential preferred
