About the role
Key responsibilities & impact- Assess clients’ needs, strengths, and goals to determine eligibility
- Develop and regularly review Individualized Service Plans (ISPs)
- Coordinate multidisciplinary care and maintain communication with stakeholders
- Ensure all documentation is accurate and compliant
- Monitor progress and adapt plans as needed
- Uphold Medicaid waiver program requirements
- Advocate for clients within healthcare and community systems
- Participate in training and quality improvement initiatives
- Collaborate with providers to deliver holistic support
Requirements
What you’ll need- Bachelor’s degree in Social Work, Psychology, Human Services, or related field
- 3+ years of case management experience (preferably with developmental disabilities or Medicaid waivers)
- Knowledge of Indiana waiver programs (HCBS), Medicaid regulations, and compliance standards
- Proficiency with EHR/case management software
- Excellent communication and organizational skills
- Valid driver’s license, reliable transportation
- Experience with person-centered and trauma-informed care
- Ability to manage multiple caseloads, prioritize, and meet deadlines
- Commitment to ethical practice and confidentiality
