About the role

Key responsibilities & impact

Assess clients’ needs, strengths, and goals to determine eligibility
Develop and regularly review Individualized Service Plans (ISPs)
Coordinate multidisciplinary care and maintain communication with stakeholders
Ensure all documentation is accurate and compliant
Monitor progress and adapt plans as needed
Uphold Medicaid waiver program requirements
Advocate for clients within healthcare and community systems
Participate in training and quality improvement initiatives
Collaborate with providers to deliver holistic support

Requirements

What you’ll need

Bachelor’s degree in Social Work, Psychology, Human Services, or related field
3+ years of case management experience (preferably with developmental disabilities or Medicaid waivers)
Knowledge of Indiana waiver programs (HCBS), Medicaid regulations, and compliance standards
Proficiency with EHR/case management software
Excellent communication and organizational skills
Valid driver’s license, reliable transportation
Experience with person-centered and trauma-informed care
Ability to manage multiple caseloads, prioritize, and meet deadlines
Commitment to ethical practice and confidentiality
