About the role

Key responsibilities & impact
  • Ensure compliance with pharmacy laws, regulations, and sterile compounding guidelines
  • Maintain IV documentation and clinical monitoring
  • Assess new patients and orders for therapy appropriateness, high-risk medications, allergies, adverse indications, interactions, and other safety risks
  • Manage prescriber/facility notifications for drug interactions, allergies, and adverse patient safety situations
  • Perform dose calculations based on medication, patient condition, volume, and administration rates
  • Program infusion pumps and clinically monitor infusion patients
  • Process medication occurrence reports
  • Follow Omnicare long-term care, home infusion, facility infusion therapy, and internal pharmacy policies and procedures
  • Process IV orders when technical support is unavailable
  • Coordinate care with home health agencies, nursing facilities, physicians, and IV nurse consultants
  • Answer drug-related IV therapy and management questions
  • Communicate with consultant pharmacists and clinical staff pharmacists regarding IV patient management
  • Orient and assess competency of pharmacists, IV technicians, and infusion pump technicians
  • Maintain knowledge of infusion devices, products, and supplies
  • Oversee cleanroom monitoring and pump maintenance
  • Follow HIPAA and other applicable government regulations
  • Perform other duties as assigned; duties may vary by location

Requirements

What you’ll need
  • Bachelor's Degree required
  • BS Degree in Pharmacy or Pharm.D. degree from an accredited School of Pharmacy required
  • Current license to practice pharmacy in good standing
  • 1 to 3 years of experience required
  • 3 years of infusion therapy experience, including clinical management of patients, required
  • 2 years of long-term care experience preferred
  • Immunization certification or willingness to become immunization certified preferred
  • Excellent organizational and time management skills
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to work independently to plan, develop, and coordinate pharmacy activities
  • Ability to read, analyze, and interpret complex topics
  • Ability to apply advanced mathematical concepts and draw valid conclusions
  • Ability to work scheduled Monday-Friday hours between 10am-6pm, with every-third-weekend rotation from 10am-6:30pm
  • Ability to meet physical demands, including prolonged sitting or standing, walking, bending, fine-print vision, normal hearing, and occasional lifting up to 25 pounds
