About the role

Key responsibilities & impact

Deliver patient care services through a remote technology platform
Independently provide holistic, evidence-based care including assessment, diagnosis, treatment, health problem management, health counseling, and disposition planning
Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices
Evaluate primary care, acute, chronically ill, and transitional care patients
Provide patient counseling, contraceptive care counseling, and medication management
Educate patients on health maintenance and respond to patient care inquiries
Document all patient care within an EHR according to MinuteClinic policies and procedures
Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home
Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members
Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

Requirements

What you’ll need

Current, unrestricted license in Florida, Ohio, Massachusetts, Rhode Island, Delaware, Maryland, or Washington, D.C.
Willingness and ability to obtain additional licensure if hired
Minimum of two years of medically relevant experience or equivalent
Effective verbal, written, and electronic communication skills
Outstanding organizational skills and ability to multitask
Initiative, problem-solving ability, adaptability, and flexibility
Ability to work without direct supervision and practice autonomously
Proficiency with information management and technology
Ability to collaborate with professional colleagues to provide quality care
Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required
Completion of a Master’s Degree level Family Nurse Practitioner program
Current National Board Certification
State of employment license to practice in the Advanced Practice Nurse role
Broadband connectivity, a quiet setting with a neutral background, and ability to uphold patient privacy for remote care delivery
Ability to work 36–40 hours per week, including every other weekend
