About the role

Key responsibilities & impact
  • Define clinical program requirements, workflow intent, and success criteria for complex care coordination programs
  • Partner with program owners across care management, acute escalations, supportive care, and other programs
  • Shape requirements and evaluate Epic and non-Epic solutions
  • Document workflows and define program success measures
  • Partner with Informatics and Product from design through go-live and ongoing optimization
  • Develop prioritized clinical requirements and use cases for technology roadmaps
  • Translate clinical program logic and workflows into actionable technical requirements
  • Drive the clinical-operations workstream of complex, multi-team launches
  • Validate delivered features through clinical acceptance and UAT
  • Develop scalable frameworks for measuring program success
  • Translate plain-language metrics into specifications for technology and data science teams
  • Monitor performance data, flag anomalies, and partner with Analytics to resolve issues
  • Represent Population Health in cross-functional forums
  • Lead, develop, and retain a high-performing team
  • Prepare executive-ready briefings, strategy documents, and slide content
  • Identify capability gaps and surface risks with recommended paths forward

Requirements

What you’ll need
  • 10–15 years in healthcare program management, population health, or care delivery operations
  • Demonstrated ownership of complex, multi-stakeholder programs
  • Deep working fluency with EHR systems; Epic strongly preferred
  • Ability to translate clinical needs into feasible, buildable requirements and assess technical tradeoffs
  • Direct build/configuration experience is a plus, not a requirement
  • Ability to influence technical and build teams as a non-owning partner
  • Comfort with clinical program design logic and technical systems, including Epic, patient segmentation data, and data warehousing concepts
  • Exceptional written communication and ability to produce executive-level materials
  • Ability to drive clinical-operations workstreams of high-stakes go-lives across matrixed teams
  • Strong analytical ability to identify data issues and determine escalation paths
  • People leadership experience developing early-career talent
  • High bias toward action and comfort with ambiguity
  • Ability to flag risks clearly and early to senior leadership
  • Bachelor's degree required
