About the role

Key responsibilities & impact
  • Evaluate members using care management assessments and information/data review
  • Recommend approaches to resolve care needs and maintain optimal health and well-being
  • Identify high-risk factors and service needs affecting member outcomes
  • Implement proactive support interventions and coordinate Wellness care plan activities
  • Monitor member care needs
  • Consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers
  • Present cases at case conferences for multidisciplinary review
  • Identify and escalate quality-of-care issues
  • Negotiate appropriate options and services to meet members’ benefits and healthcare needs
  • Use motivational interviewing to engage members and promote lifestyle and behavior changes
  • Coach and educate members to support independent medical and healthy lifestyle choices
  • Help members participate knowledgeably in healthcare decision-making with providers
  • Apply case management and quality management processes in compliance with regulatory, accreditation, and company requirements

Requirements

What you’ll need
  • Minimum 6 months of experience in foster care and juvenile justice systems, adoption assistance, behavioral health services, trauma-informed care, ACEs, crisis intervention services, or applicable evidence-based practices for Kentucky SKY populations
  • Minimum 1 year of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling
  • Minimum 1 year of experience with Microsoft Office Suite applications, including Word and Excel
  • Willingness and ability to travel 80-90% of the time to meet members face-to-face in the assigned region
  • Reliable transportation required
  • Must reside in Jefferson County, Kentucky
  • Minimum of a bachelor’s degree or a non-licensed master-level clinician credential in a behavioral health or human services field
  • Flexibility to work beyond Monday-Friday, 8am-5pm EST, including after-school and after-work hours
