Enable equitable, coordinated care by strengthening community and provider ecosystems
Improve care navigation and equip the workforce to deliver integrated, culturally responsive care
Build and steward trusted relationships with communities, providers, and partners
Identify and build opportunities to increase member and provider engagement
Create models for and execute on opportunities to enable community workforce to deliver integrated, culturally responsive care
Ensures enterprise colleagues and community workforce have the necessary skills, tools, and confidence to support equitable care navigation (training, education, etc.)

Requirements

What you’ll need

8-10 years of prior relevant work experience
Population health program implementations
Demonstrated experience in building interventions to address upstream barriers for better health outcomes
Prominent level of interpersonal skills and ability to manage multiple business areas, community and provider partners
Coordinates and prepare for external community partners and provider meetings
Ensures that the most impactful internal subject matter experts (clinical, regulatory, financial, analytical, etc.) are utilized to optimize performance
Experience developing analyses and presentations to explain business issues, quantify financial impact, and provide recommendations
Exceptional oral, written, and presentation communication skills
Attention to detail and proactive problem‑solving
Proven ability to interact with, influence and collaborate with internal and external stakeholders at all levels
Experience managing matrixed environment with ability to leverage internal business partners to complete intervention design
