Conduct online “Video Visits” with health plan members
Engage with health plan members primarily online, helping to close gaps in care
Provide 40 hours of availability weekly
Execute a strong visit completion rate
Maintain compliance with operational, clinical, and member experience key performance indicators

Requirements

What you’ll need

Full-time, board-certified nurse practitioners or physician assistants
Experience with Signify Health In-Home Health Evaluations, Video Visits, telehealth visit delivery
Maintain training, affiliation, and compliance with Video Visit requirements
Be trained and credentialed for all health plan clients in primary coverage state(s)
Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
Able to obtain and maintain multi-state licenses as required
