This Case Manager Registered Nurse position is with Aetna’s Long-Term Services and Supports (LTSS) team
Field-based position out of Lake County IL
Coordinate all case management activities with members to evaluate medical needs
Facilitate member’s overall wellness
Develop a proactive course of action to address issues

Requirements

What you’ll need

Minimum 2+ years of clinical experience required
Active and unrestricted RN license in the state of IL
Must reside in Lake County IL
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time
Case management and discharge planning experience preferred
Managed Care experience preferred
Microsoft Office Suites experience preferred
Minimum of Associate’s Degree in Nursing required
Bachelor’s Degree in Science and Nursing preferred
