Key responsibilities & impact

support optimal member health outcomes by coordinating care, providing education, and facilitating access to appropriate healthcare services
assess member needs, identify risks, and develop and implement effective care plans
collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits
promote member engagement, informed decision-making, and healthy behavior changes
monitor care plan progress, documenting activities, and ensuring compliance with regulatory standards

Requirements

What you’ll need

2 or more years of behavioral health, social services or human services field experience
2 or more years’ case management experience
Must possess reliable transportation and be willing and able to travel up to 75% of the time
Discharge Planning and/or Managed Care (MCO) experience
Microsoft Office Suite Experience
Bachelor’s Degree or non-licensed Master’s level clinician required
Degree to be in behavioral health, human services (psychology, social work, marriage and family counseling)
