About the role

Key responsibilities & impact

This is a full-time telework role for a CDCES registered nurse
Candidate must reside in a compact state and hold a compact license
Must have experience working with patients with diabetes
Working schedule: Monday-Friday, standard business hours, including 2 evening shifts per week from 11:30am-8pm CST
Rotating Fridays and Saturdays will be required approximately two times per quarter following training
The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote and influence members' decisions related to achieving and maintaining optimal health status for chronic conditions
Assessment of members through the use of clinical tools and information/data review, conducting comprehensive evaluation of members' needs and benefit plan eligibility for available integrated internal and external programs/services
Utilizes assessment techniques to determine members' level of health literacy, technology capabilities, and/or readiness to change
Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices

Requirements

What you’ll need

Active and unrestricted RN licensure in a compact state, holding a compact license
3+ years of clinical experience
Must possess or be willing and able to obtain high speed broadband internet access
Previous coaching experience (preferred)
Managed care experience (preferred)
Experience with Microsoft Word, Outlook, Excel, and comfortable using various computer programs (preferred)
