About the role

Key responsibilities & impact

The Care Manager—Registered Nurse is responsible for coordinating care for members with chronic conditions and SDoH needs.
Conduct comprehensive assessments and connect members to appropriate resources.
Develop and implement individualized care plans, monitor member progress, and advocate for services.
Document assessments and interventions in the electronic health record.
Collaborate with interdisciplinary care team to address barriers and develop maintenance strategies.

Requirements

What you’ll need

Must have active and unrestricted Registered Nurse (RN) licensure in the state of MI OR compact licensure in state of residence
Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
Minimum 3+ years of nursing experience
Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience
Experience providing care management for Medicare and/or Medicaid members (preferred)
Bilingual skills, especially English-Spanish (preferred)
