About the role

Key responsibilities & impact

Deliver high-quality case management services through telephonic and face-to-face engagement
Conduct comprehensive telephonic and in-person assessments to evaluate members' medical and behavioral health needs
Develop and implement individualized care plans
Coordinate and monitor ongoing care plan activities
Collaborate in multidisciplinary care reviews
Identify and escalate quality of care concerns
Engage members using motivational interviewing and coaching techniques
Educate and empower members to participate in healthcare decision-making
Ensure accurate and timely documentation
Serve as a preceptor and mentor to new nurses
Support onboarding by teaching policies, workflows, and best practices

Requirements

What you’ll need

Active, unrestricted Registered Nurse (RN) license
Must reside in the following area: Fort Worth, Arlington, Edgecliff Village, Richland Hills, Kennedale and Forest Hill, TX
Minimum 3+ years of clinical experience
At least 1+ years of case management experience
Must possess reliable transportation and be willing and able to travel up to 75% of the time.
