About the role

Key responsibilities & impact

Manage member care through telephonic assessments and case management
Develop personalized care plans to improve health outcomes
Identify risks, remove care barriers, and support complex health needs
Coordinate with multidisciplinary teams to ensure seamless care
Use clinical expertise and data insights to guide decision-making
Schedule Monday–Friday, 9:00 AM–5:30 PM (based on your time zone)
Occasional evenings, weekends, or holidays as needed

Requirements

What you’ll need

Active Illinois RN license (required; no IL residency needed)
2+ years of RN clinical experience (all specialties welcome)
Strong communication, assessment, and care coordination skills
Associate’s Degree in Nursing
Case management or managed care experience (Nice to Have)
CCM certification (Nice to Have)
BSN (Nice to Have)
Advanced proficiency with Microsoft Office (Nice to Have)
