Key responsibilities & impact
  • Support case management team in delivering high-quality, member-centered care
  • Apply critical thinking and sound judgment to collaborate across disciplines
  • Coordinate care, provide education and support, and connect members to resources

  • Monitor and evaluate care plans, ensuring compliance with regulatory standards

Requirements

What you’ll need
  • Candidate must reside in Vienna, Alexandria, Arlington, Annandale, Mclean, Sterling or Reston, VA or surrounding areas (within 15 minutes)
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time in the Northern Virginia region.
  • 2-4 years of experience in behavioral health, social services or appropriate related field equivalent to program focus
  • Must be comfortable working in a fast-paced environment
  • Bilingual
  • Experience with home visitation
