Key responsibilities & impact

Assessing, planning, and coordinating comprehensive care for members with diverse medical and behavioral health needs
Conducting in-depth, face-to-face assessments to identify member needs
Developing and implementing individualized care plans
Coordinating medical, behavioral, and community-based services

Requirements

What you’ll need

Must reside in Norfolk, VA area (Chesapeake, Portsmouth)
Must possess reliable transportation and be willing to travel up to 75% of the time
RN/BH with current unrestricted Virginia state licensure required
3 years Clinical Experience (Preferably in Medical-Surgical Nursing, Behavioral Health, Long term Care, etc.)
1+ years Managed Care experience
Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding
