Key responsibilities & impact
  • Build trusted relationships with members through in-person visits (approximately 70% travel)
  • Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs
  • Develop and implement individualized care plans that promote health, independence, and quality of life
  • Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, high-quality care
  • Connect members to services, supports, and resources that align with their goals
  • Ensure care delivery meets regulatory requirements and Aetna quality standards
  • Advocate for medical appropriateness while supporting positive member engagement and outcomes

Requirements

What you’ll need
  • 2+ years of experience in behavioral health, social services, or a related field aligned with program focus
  • Residency in one of the following Virginia areas: Alexandria, Arlington, Vienna, McLean, Woodbridge, and/or Springfield
  • Reliable transportation and the ability to travel up to 70% of the time (mileage reimbursed per company policy)
