- Assessing, planning, implementing, and coordinating all case management activities with members
- Manage a caseload of supportive and medically complex members
- Develop proactive courses of action to enhance member outcomes
- Determine appropriate services and supports for members' health needs
Requirements
- Bilingual in English & Spanish
- 1+ year of experience in behavioral health or a long-term care setting
- 1+ year of experience in case management and discharge planning experience
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in Melbourne and surrounding areas
