About the role

  • Assessing, planning, implementing, and coordinating all case management activities with members
  • Evaluating the medical needs of the member to facilitate the member's overall wellness
  • Managing a caseload that includes supportive and medically complex members
  • Developing a proactive course of action to address issues presented to enhance the outcomes
  • Determining appropriate services and supports due to member's health needs

Requirements

  • Bi-lingual in English and Spanish
  • 1+ year of experience in behavioral health or a long-term care setting
  • Computer proficiency in Microsoft Word, Excel, and Outlook
  • Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL
  • Must possess reliable transportation and be willing and able to travel up to 50-75% of the time
