About the role
- Coordinating care for members with chronic medical and behavioral health conditions
- Conducting comprehensive assessments and connecting members with resources
- Developing and implementing individualized care plans
- Monitoring member progress and advocating for necessary services
- Documentation of assessments and interventions in electronic health record
- Collaborating with interdisciplinary care team to ensure optimal health outcomes
- Engaging with members telephonically to manage their care
Requirements
- Active and unrestricted Registered Nurse (RN) licensure in the state of Kentucky (KY) OR compact licensure in state of residence
- Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
- Access to a private, dedicated space to conduct work effectively
- Confidence working at home / independent thinker
- 3+ years of nursing experience
- 2+ years of case management, discharge planning and/or home healthcare coordination experience
- Bilingual skills, especially English-Spanish (Preferred)
