About the role
- Responsible for telephonically and/or face-to-face assessing, planning, implementing, and coordinating case management activities with members
- Evaluate the medical needs of members to facilitate overall wellness
- Manage a caseload that includes supportive and medically complex members
- Develop proactive courses of action to address issues to enhance outcomes
- Determine appropriate services and supports due to member's health needs including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports
Requirements
- Must reside in Melbourne FL, or surrounding areas such as Palm Bay and Satellite Beach FL
- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face
- 1+ year of experience in behavioral health or a long-term care setting
- 1+ year of experience in case management and discharge planning experience
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Bilingual in English & Spanish preferred
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
- Managed care experience
