About the role
- Execute clinical pharmacy programs
- Coordinate strategies for increasing adherence
- Utilize motivational interviewing and health coaching to impact member behavior
- Outreach to members 90% of the day
- Ensure effective collaboration with internal and external matrix partners to support clinical programs
- Work collaboratively with leadership and management to strengthen business and operational processes
- Other duties as required
Requirements
- 1+ years of experience with Medicare Stars, managed care or population health
- 2+ years of customer or patient-facing experience
- Enhanced knowledge of pharmacy and claims processing
- Excellent organizational and analytical skills
- Superior communication skills with patients and clinicians
- Ability to prioritize daily to monthly deliverables
- Computer Skills: Proficient in Microsoft Excel, Microsoft Teams, and Sharepoint
