About the role
- Utilizes critical thinking and judgment to collaborate and inform the case management process
- Conducts comprehensive evaluation of referred member’s needs/eligibility
- Recommends an approach to case resolution and/or meeting needs
- Coordinates and implements assigned care plan activities
- Monitors care plan progress
- Consults with case managers, supervisors, Medical Directors and/or other health programs
- Identifies and escalates quality of care issues
- Utilizes negotiation skills to secure appropriate options and services
- Provides coaching, information and support to empower the member
- Helps member actively and knowledgably participate with their provider in healthcare decision-making
- Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines
Requirements
- Candidate must reside in Little Village or North Lawndale IL
- 2 years’ experience in behavioral health, social services, or appropriate related field equivalent to program focus
- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time within Westside Chicago, IL
- Case management and discharge planning experience preferred
- Managed care experience preferred
- Microsoft Office experience preferred
- Bachelors Degree prepared in human-services related field
- Masters Degree in human-services related field preferred
