About the role
- Utilize collaborative process of assessment, planning, implementation and evaluation to engage, educate, and promote/influence members' decisions related to achieving and maintaining optimal health status
- Work from home, requiring reliable phone and hard-wired internet service
- Work schedules determined based on business needs
- Must be reachable via phone, videoconference, and email during work hours
- Maintain confidentiality (HIPAA compliant)
- Meet additional reasonable requirements set by the Leader or Company
Requirements
- Active unrestricted RN compact state licensure
- 3+ years of clinical experience required in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, telehealth and/or Oncology
- Proficient in Multisystem Navigation
- Effective Multitasker
- Basic Technical Troubleshooting
- Digital Collaboration & Documentation
- Highly Organized & Results-Oriented
- Metric-Driven Performance
- Managed Care experience preferred
- Previous coaching experience preferred
- Telephone triage nursing experience preferred
- Associate Degree required
- Bachelor's Degree or Equivalent 3-5 years clinical experience preferred
