About the role
- Assessing, planning, implementing, and coordinating all case management activities with members
- Evaluating the medical needs of the member to facilitate their overall wellness
- Managing a caseload that includes supportive and medically complex members
- Developing a proactive course of action to address issues to enhance member's overall wellness through integration
- Determining appropriate services and supports due to member's health needs including Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports
Requirements
- Bi-lingual in English and Spanish
- 1 + year of experience in behavioral health or a long-term care setting
- Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL
- Must possess reliable transportation and be willing to travel 50-75% of the time to meet members face to face in North Miami FL
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Bachelor's degree required, preferably in Social Work or related field required
- Master's degree preferred
