About the role
- Assessing, diagnosing, and treating individuals with serious mental health and substance use disorders
- Support treatment plan updates related to medication and monitor Medication Administration Records (MARs)
- Provide a weekly medication group and be available for impromptu phone calls regarding medication questions
- Conduct groups as needed to support clients with medication management and skill development
- Hold weekly individual meetings with all clients to address needs and progress
- Complete documentation within 24 hours and meet KPIs monthly, quarterly, yearly
- Provide periodic UA sign-off and crisis support services at Hazelbrook Community Center in partnership with peer support team
Requirements
- Active and unrestricted PMHNP license in the state of Colorado
- Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC)
- Ability to communicate and work effectively with the Clinical Director and Program Director regarding treatment plans
- Availability to work a minimum of two times a week
- Strong problem-solving skills and ability to handle crisis situations effective
- Sensitivity to and understanding of clients from diverse backgrounds
- Experience with Employee Health Records (EHR) systems and online medical records
- Behavioral health & substance abuse experience
- (Nice to have) Group facilitation experience, particularly for substance abuse management
- (Nice to have) Bilingual in Spanish and English
