Case Manager – Community-Based Case Manager
Community CorporateCommunity-Based Case Manager at Community Empowerment Resources supporting individuals with mental illness through recovery-oriented care. Building connections and providing coordinated services in the community.
About the role
Key responsibilities & impact- Develop and maintain therapeutic, recovery-oriented relationships
- Create and implement individualized service and recovery plans
- Coordinate psychiatric, medical, housing, legal, vocational, and substance use services
- Provide support in homes and community-based settings
- Transport clients when clinically appropriate and safe
- Collaborate with treatment providers and community partners
- Maintain accurate and timely documentation in compliance with agency and funding requirements
Requirements
What you’ll need- Preferred: Bachelor’s degree in Psychology, Social Work, Health, Nursing, Counseling, or related field (with coursework in psychology)
- Required: 6–12 months of supervised experience (employment, internship, volunteer, or combination) working with at least one of the following populations: SPMI, DD, Elderly, Children or other special needs population.
- A combination of relevant education and experience accepted
- Strong time management, communication and organizational skills
- Comfortable working independently and autonomously in community settings
- Required: Access to a reliable vehicle, valid driver’s license, clean driving abstract, and valid vehicle safety check
