About the role
- Conduct comprehensive in-home client assessments
- Develop, implement, and update individualized care plans
- Monitor ongoing care and adjust plans as client's needs change
- Provide clinical guidance and support during care transitions or urgent situations
- Coordinate care with clients, families, Caregivers, and healthcare providers
- Represent Commonwise Home Care within the Williamsburg community
- Build and maintain relationships with referral partners and community organizations
- Participate in local events, meetings, and educational activities
- Collaborate with internal teams to support local growth and referral efforts
- Assist with caregiver recruitment and onboarding efforts as needed
- Provide clinical training, mentorship, and ongoing support to Caregivers
- Conduct performance check-ins to ensure care quality and adherence to standards
- Collaborate with the Care Coordination team to align clients and Caregivers effectively
- Maintain regular communication with clients and families to assess satisfaction
- Address concerns and support positive care experiences
- Partner with internal teams to support care delivery and operational needs
- Serve as the local clinical resource for the Williamsburg area
- Participate in team meetings and contribute to process improvement efforts
Requirements
- Active RN license in the state of Virginia
- Minimum of 5 years of nursing experience
- Strong communication and relationship-building skills
- Ability to work independently and manage multiple priorities
- Comfort working in community-based and home settings
