About the role

Key responsibilities & impact
  • Manage a panel of clinically complex members supporting clinical programs, quality gap initiatives, and ED/IP utilization.
  • Collaborate with members to create care plans, oversee progress, reassess needs, and support program or pathway graduation.
  • Coordinate with the integrated Cityblock Care Team, external providers, and community partners.
  • Communicate program expectations, including duration and goals, to engaged members.
  • Complete self-efficacy, condition-specific, and behavioral health screeners, including PHQ-9, GAD-7, AUDIT, and DAST-10.
  • Refer members to behavioral health programs as needed.
  • Conduct in-person clinical examinations and help determine member placement in programs of varying intensity.
  • Prepare for and participate in case conferences, leading discussions when necessary.
  • Develop care plans, address social needs with Community Health Partner support, and coordinate comprehensive care.
  • Conduct regular clinical visits and follow-ups, monitor therapeutic interventions, and address member needs.
  • Support health maintenance, chronic disease management, and co-occurring psychiatric disorder care.
  • Perform medication reconciliation, administration, compliance, and education.
  • Address prioritized quality gaps and maintain thorough chart documentation and ICD/CPT coding.
  • Use care facilitation tools, electronic health records, and scheduling platforms to gather data, document interactions, track tasks, and communicate with resources.
  • Support members through inpatient and emergency department transitions of care.

Requirements

What you’ll need
  • Graduate of an accredited school of nursing (R.N.)
  • 3+ years of experience
  • Strong critical thinking and sound clinical judgment
  • Ability to make complex decisions independently and recognize when to collaborate
  • Ability to identify system barriers and develop creative, practical solutions
  • Strong written and verbal communication across phone, text, virtual, and in-person settings
  • Comfortable using technology to engage members remotely
  • Knowledge of Motivational Interviewing and Trauma-Informed Care principles
  • Ability to translate clinical information for non-clinical audiences and actively listen
  • Medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or an approved medical or religious accommodation
