About the role
Key responsibilities & impact- Provide behavioral health support to members in in-person and virtual settings
- Develop member care plans when behavioral health interventions are needed
- Provide clinical education on preventative care topics, including behavioral health and mental illness
- Refer members to medical support, social resources, behavioral health, and Substance Use Disorder providers or care teams
- Collaborate with internal care teams to assess member escalation needs or placement in different-intensity programs
- Maintain engagement with high-acuity members
- Triage member needs through care team referrals
- Provide short-term therapeutic interventions
- Address members presenting with serious mental illness or substance use disorder
- Manage behavioral health escalations and facilitate transitions of care as needed
Requirements
What you’ll need- Active, unrestricted LCSW license in good standing
- 1 year of experience post Master’s Degree
- Completion of Cityblock medical clearance requirements for member-facing roles, including evidence of immunity to MMR, Hepatitis B, and Varicella, and a TB screen, unless an approved medical or religious accommodation applies
- Availability Monday–Friday, 8:30 a.m.–5:00 p.m.
- Ability to work two days per week onsite at the Charlotte Hub and three remote workdays
- Ability to provide care in in-person and virtual settings
