About the role

Key responsibilities & impact

Triage member needs through care team referrals
Provide short-term therapeutic interventions
Address members presenting with serious mental illness or substance use disorder
Manage behavioral health escalations
Facilitate transitions of care as needed
Provide behavioral health support in in-person and virtual settings
Develop member care plans when behavioral health interventions are needed
Provide clinical education on preventative care topics, including behavioral health and mental illness
Refer members to medical support, social resources, behavioral health, and substance use disorder providers or internal care teams

Requirements

What you’ll need

Active New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) credential
1 year of experience post-Master’s Degree
Medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or an approved medical or religious accommodation
Ability to provide behavioral health support in in-person and virtual settings
Experience or ability to address serious mental illness and substance use disorder needs
Ability to develop member care plans and provide short-term therapeutic interventions
Ability to make appropriate referrals and collaborate with internal care teams
Ability to maintain engagement with high-acuity members
