About the role
Key responsibilities & impact- Triage member needs through care team referrals
- Provide short-term therapeutic interventions
- Address members presenting with serious mental illness or substance use disorder
- Manage behavioral health escalations
- Facilitate transitions of care as needed
- Provide behavioral health support in in-person and virtual settings
- Develop member care plans when behavioral health interventions are needed
- Provide clinical education on preventative care topics, including behavioral health and mental illness
- Refer members to medical support, social resources, behavioral health, and substance use disorder providers or internal care teams
- Collaborate with internal care teams to assess escalation needs or placement in different programs

Requirements
What you’ll need- Active New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) credential
- 1 year of experience post-Master’s Degree
- Medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or an approved medical or religious accommodation
- Ability to provide behavioral health support in in-person and virtual settings
- Experience or ability to address serious mental illness and substance use disorder needs
- Ability to develop member care plans and provide short-term therapeutic interventions
- Ability to make appropriate referrals and collaborate with internal care teams
- Ability to maintain engagement with high-acuity members
