About the role
Key responsibilities & impact- manages a member panel to support care coordination and social care for high-complexity members
- conducts assessments and screening instruments
- collaborates with psychiatrists, RNs, and Behavioral Health Specialists for case review and care planning
- meets with members in the community as needed
- utilizes care facilitation, electronic health record, and scheduling platforms as needed
Requirements
What you’ll need- 2+ years of progressive experience in care coordination, community health, health promotion, or a related field
- working with individuals experiencing behavioral health conditions (e.g., substance use disorders or serious mental illness) in a case management or comparable role
