About the role
Key responsibilities & impact- Call a full caseload of Medicare patients with two or more chronic conditions monthly using specialized care management software
- Build and maintain rapport with patients through SMART goals and self-management education
- Implement and improve patient Plans of Care by updating medications, appointments, biometrics, symptoms, and interventions
- Connect patients with community resources, including transportation, personal care, prescription/DME assistance, and social services
- Conduct Transitional Care Management activities for high-risk patients discharged from hospitals and emergency rooms
- Close preventive care gaps by encouraging wellness visits, vaccinations, cancer screenings, and follow-up or specialist appointments
- Provide accurate, timely, and complete documentation
- Prioritize and close case tasks on schedule while meeting quality, compliance, and performance expectations
Requirements
What you’ll need- Fluent in English
- Current, unrestricted Illinois RN license required
- 3 or more years' experience as a Registered Nurse
- Proficiency with electronic health records and web-based applications
- Self-directed and able to work independently with little supervision while meeting performance metrics
- Passion for nursing and improving patient outcomes
- Good with technology and eager to learn and use new software
- Excellent organizational and time management skills
- Strong communication and telephonic skills
- Strong critical thinking and problem-solving skills
- Strong internet-connected computer; equipment is not provided by the company
- Minimum 20 hours of daytime availability per week between 9am and 6pm Eastern, Monday-Sunday
- Ability to commit to own schedule using company software
- Responsible for own equipment, taxes, and insurance as a 1099 contractor
