About the role

Key responsibilities & impact
  • Improve the overall health of the communities we serve by providing individualized nutritional support to patients for their particular situation.
  • Meet directly with patient to identify dietary needs through nutritional assessment based on nutrition-focused physical findings and patient history.
  • Provide nutritional counseling either individually or in a group setting; identify goals and support education with evidence-based care.
  • Develop detailed care plans and document as part of the electronic health record in accordance with established program standards.
  • Assess, plan and implement the delivery of medical nutrition therapies in collaboration with the patients inter-disciplinary team.
  • Monitor and evaluate patient's progress through routine nutrition interventions to support positive health outcomes.
  • Participate and support patients in various program committees; including developing educational programs for various ethnic, cultural and social groups to teach nutrition education.
  • Remain up to date on evidence-based research and recommend ways to expand and improve nutrition programs.
  • Participate in quality improvement of the program in conjunction with other services CHAS Health offers.

Requirements

What you’ll need
  • Bachelor’s degree in Nutrition or related field required.
  • Registered Dietitian credentialed with the Commission on Dietetic Registration and eligible for state licensure.
  • Experience in chronic disease including HIV and Diabetes and/or maternity nutrition counseling preferred.
  • Excellent oral and written communications skills required.
  • Computer skills required.
  • BLS (CPR/AED) required.
