About the role

Key responsibilities & impact

Provide administrative support for the Foster Care/DCYF team
Assist Care Managers and Care Navigators with emailing and updating monitoring care plans
Maintain assigned monitoring care plans
Complete member outreach to support comprehensive assessments
Connect members to community resources supporting their care management journey
Provide members with known community resources and health education as appropriate
Support members in connecting to community and care resources
Coordinate community outreach resources and promote awareness of care/services
Respond to member inquiries about community and care resources
Support member correspondence and educational materials
Escalate community resource requests and inquiries to management
Document and maintain community resources in accordance with health plan requirements
Support nurses and social workers with outreach, community education, evidence-based guidance, and member support
Conduct non-clinical general health assessments and refer members to appropriate services
Conduct non-medical assessments and resolve concerns on members’ behalf
Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families
Make visits to individual homes and/or community organizations as needed
Perform other duties as assigned

Requirements

What you’ll need

High School diploma or GED
2 - 4 years of related experience
Working knowledge of Social Determinants of Health (SDOH) barriers
Ability to conduct non-clinical general health assessments
Ability to conduct non-medical assessments, including home safety, community/environment resources, transportation, and employment
Ability to conduct telephonic and/or in-person outreach
Compliance with all policies and standards
Must follow health plan requirements and standards of practice
