About the role

Key responsibilities & impact
  • Provide administrative support for the Foster Care/DCYF team
  • Assist Care Managers and Care Navigators with emailing and updating monitoring care plans
  • Maintain assigned monitoring care plans
  • Complete member outreach to support comprehensive assessments
  • Connect members to community resources supporting their care management journey
  • Provide members with known community resources and health education as appropriate
  • Support members in connecting to community and care resources
  • Coordinate community outreach resources and promote awareness of care/services
  • Respond to member inquiries about community and care resources
  • Support member correspondence and educational materials
  • Escalate community resource requests and inquiries to management
  • Document and maintain community resources in accordance with health plan requirements
  • Support nurses and social workers with outreach, community education, evidence-based guidance, and member support
  • Conduct non-clinical general health assessments and refer members to appropriate services
  • Conduct non-medical assessments and resolve concerns on members’ behalf
  • Conduct telephonic and/or in-person outreach to locate hard-to-reach individuals and families
  • Make visits to individual homes and/or community organizations as needed
  • Perform other duties as assigned

Requirements

What you’ll need
  • High School diploma or GED
  • 2 - 4 years of related experience
  • Working knowledge of Social Determinants of Health (SDOH) barriers
  • Ability to conduct non-clinical general health assessments
  • Ability to conduct non-medical assessments, including home safety, community/environment resources, transportation, and employment
  • Ability to conduct telephonic and/or in-person outreach
  • Compliance with all policies and standards
  • Must follow health plan requirements and standards of practice
