About the role
Key responsibilities & impact- Develop, assess, and coordinate holistic care management activities to support quality, cost-effective healthcare outcomes
- Develop or assist with personalized service care plans for long-term care members
- Educate members and families/caregivers about services, benefits, procedures, healthcare instructions, referrals, and service options
- Evaluate member needs and available resources, and facilitate appropriate care plans
- Identify providers, specialists, community resources, and durable medical equipment services
- Coordinate between members/families/caregivers and care provider teams to ensure services are accessible
- Provide resource support related to employment, housing, participant direction, independent living, justice, and foster care
- Monitor care plans, member status, and outcomes; recommend updates based on member needs
- Interact with long-term care providers and partners to ensure member needs are met
- Collect, document, and maintain member information and care management activities for regulatory compliance
- Perform in-home or on-site visits to assess member needs and collaborate with providers and resources
- Provide feedback to leadership on opportunities to improve quality and cost-effective service delivery
- Perform other duties as assigned and comply with policies and standards
Requirements
What you’ll need- Bachelor's degree and 1 year of related experience
- For the Illinois Plan, one of the following: RN licensed in Illinois; bachelor's or master's degree in a human services-related field; or LPN with one year of experience conducting comprehensive assessments and providing formal services for the elderly
- Human services-related fields include child, family and community services; early child development; guidance and counseling; home economics—child and family services; human development counseling; human service administration; human services; Master of Divinity; pastoral care; pastoral counseling; psychiatric nursing; psychiatry; psychology; public administration; rehabilitation counseling; social science; social services/social work; or sociology
- Past case management, advocacy, or home visits/community travel experience preferred
- Ability to conduct in-home assessments and coordinate care plans
- Ability to connect members with healthcare providers, community resources, and DME services
- Ability to work Monday through Friday, 8:00 a.m.–4:30 p.m. CT with a 30-minute lunch
- Ability to perform 50% local travel and on-site visits
- Compliance with applicable policies, standards, and regulatory requirements
