About the role
- Assesses, plans, and implements complex care management activities based on member activities to enable quality, cost-effective healthcare outcomes
- Develops personalized care plans for care members
- Coordinates and manages between members and providers to ensure person-centered care
- Monitors care plans and collaborates with appropriate providers to revise care plans as necessary
- Engages with new hires and provides guidance during onboarding
Requirements
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 4 – 6 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
