Develops personalized care plans for care members
Coordinates and manages between members and providers to ensure person-centered care

Requirements

What you’ll need

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
4 – 6 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
